State when you started working at that establishment, your job title, the location of the asbestos and the date(s) during which you where exposed.

You are most likely to be exposed to asbestos by breathing in asbestos fibres that are suspended in air. Workers who are making repairs or installations in buildings with asbestos-containing materials may be exposed to higher levels of asbestos. Since vermiculite and talc may contain asbestos, occupational workers and the general population may be exposed to asbestos when using these products.

Although the harm caused by asbestos is not apparent at first, asbestos exposure can lead to serious, debilitating, and often fatal diseases. Exposure to asbestos and related asbestos diseases pose serious risks to your health.
